We walked through Tmates’ full GLP-1 intake ourselves, from the welcome screen through four sequential safety multi-selects to the no-account-required final checkout. Here’s what an actual prospective patient encounters before being prescribed.

The intake doesn’t use a validated PHQ-2 or PHQ-9 depression screen β suicidal thoughts and depression are checkboxes in the safety multi-selects. There is no dedicated eating-disorder screen with severity grading (anorexia / bulimia were not visible in our walkthrough). The allergy capture is a single Yes/No on “allergic to any medications” with no follow-up list for food, dyes, supplements, or specific drugs. Current medications are a Yes/No only β no medication list is collected. There is no diet or exercise history beyond a Yes/No on prior weight-management programs (Jenny Craig, WW). Sex is collected as a binary Male / Female only.
Tmates is the only compounded GLP-1 provider in our catalog so far that lets a prospective patient complete the entire 24-screen clinical questionnaire β including all four safety multi-selects, BP and HR ranges, and the eligibility reveal β without entering an email, phone number, or creating an account. Contact information is collected on the very last screen right before “Proceed to Checkout.” For comparison, providers like Yucca Health drop an email gate at pre-qualification, and Strut Health requires email + phone (or Google SSO) before any clinical question. The trade-off: there’s no progress-saving link emailed to the patient mid-flow, so an interrupted session means starting over.
