What the Synergy Rx Intake Looks Like
We walked through Synergy Rx’s full GLP-1 intake ourselves, from the BMI calculator through two condition multi-selects to the approve-first-charge checkout. Here’s what an actual prospective patient encounters before being prescribed β including a notable FDA-black-box gap and a contradictory consultation-fee disclosure.

Notably weaker than peer compounded providers. Synergy Rx asks two condition multi-selects with a suicidal-thoughts checkbox and the cancer 5-year-remission threshold, but MTC and MEN2 family history are not asked anywhere in the visible flow β a significant FDA-black-box gap. There is also no allergy screen, no eating-disorder check, no medication list, and no BP/HR reading.

Low. Anonymous-friendly questionnaire β name, email, and phone aren’t collected until after every clinical question. Approve-first-charge applies at the plan-selection step, but the consultation-fee disclosure contradicts itself ($80 on one screen, $59 on another) and the brand identity is muddled (RxActivate appears at checkout instead of Synergy Rx).

Clinical Safety Screens Performed
Medullary Thyroid Cancer (MTC) and Multiple Endocrine Neoplasia type 2 (MEN2) family history are the GLP-1 class FDA black-box warning, and most compounded providers in our catalog ask about them as a dedicated checkbox or a multi-select item. Synergy Rx does not ask about MTC or MEN2 anywhere in the visible 16-screen flow β neither personal history nor family history. The intake also doesn’t include pancreatitis, gastroparesis, anorexia or bulimia, an opiate or alcohol screen, an allergy screen of any kind, or a medication list (current meds is a Yes/No only). Sex is collected as a binary Male / Female only, and no actual blood pressure or heart-rate reading is captured.
The TeleHealth-visit fee surfaces in two contradictory amounts. The billing-cycle screen states: “There is an $80 TeleHealth visit cost regardless of whether I receive medication.” The Care Validate disclosure on the final payment page reads: “You will be charged $59 for your initial TeleHealth visit.” Patients should not assume either amount is the binding figure β both quotes are visible in the same flow.
The Care Validate copy on the final screen also references “RxActivate remains your primary service provider” rather than “Synergy Rx,” suggesting RxActivate is the underlying entity operating multiple consumer brands on the same Care Validate billing infrastructure. Patients may see CAREVALIDATE or RxActivate on card statements rather than Synergy Rx.
