Hims’ intake does not collect any real diet-history or weight-loss-history depth — there’s no question about specific diets tried, programs attempted, past prescription weight medications, or how long prior attempts lasted. Physical activity is captured as a single 5-point scale. It also uses a simpler binary question about harm rather than a formally validated depression instrument like PHQ-2 or PHQ-9.
