The intake itself collects minimal clinical data — one metabolic-conditions multi-select, one recent-medication question, and a BMI calculation. Mochi relies on the live video provider visit (scheduled after signup) for the deeper screening.

The flow itself is quick. The main friction sits at the bookends: account creation is required on screen 1, and the final step is a tiered recurring membership commitment before the live visit.
The 7 Stages
Mochi’s eligibility flow is intentionally short because the clinical screening is performed during the scheduled video visit with a provider — not inside the intake form. The intake is designed to collect the minimum viable information (identity, BMI, recent med use, metabolic conditions) and hand the rest of the medical history to a live clinician. For patients who prefer talking through their history with a human rather than filling out a long form, this is a genuinely different model. For patients who want the full medical history captured before the visit, the depth comes after the checkout step.
The pre-visit intake does not include an eating-disorder screen, a mental health or depression question, an FDA black-box family history (MTC, MEN2, pancreatitis), a substance-use or alcohol screen, an allergy check, a full medication review beyond weight-loss drugs, or any diet or exercise history. Blood pressure isn’t measured or asked. These topics are expected to surface during the live provider visit rather than in the intake itself. Patients who don’t want to spend much time filling out forms before talking to a clinician may appreciate this structure; patients who want their provider to review a complete medical profile in advance will want to prepare that information to share verbally.
