Our Review
Tmates earns a 3.5 out of 5 from our editorial team, buoyed by its affordable pricing, insurance acceptance, diverse medication options, and strong ongoing support. The main concern is a near-total lack of independent reviews β€” its Trustpilot page shows just 1 review. This provider is best suited for budget-conscious patients who want insurance coverage options and appreciate hands-on support, and who are willing to be early adopters of a less-reviewed platform.

About Tmates

Tmates has been operating since 2022 and offers one of the broadest medication menus we have encountered, including both brand-name options like Wegovy and Zepbound as well as compounded semaglutide, tirzepatide, and phentermine. What immediately stands out is the pricing: at $99 to $167 per month, Tmates is among the most affordable providers in the GLP-1 telehealth space, particularly impressive given its inclusion of brand-name medications. The platform also accepts insurance, which could significantly reduce costs for eligible patients.

From a support perspective, Tmates distinguishes itself with high ongoing support β€” a rating we do not assign lightly in our reviews. This means patients can expect consistent check-ins, accessible customer service, and a more hands-on experience than the typical “prescribe and forget” telehealth model. This level of accreditation adds a layer of accountability that many competitors lack.

The inclusion of phentermine alongside GLP-1 medications is an unusual offering that broadens Tmates’ appeal. Phentermine is an older, well-established weight loss medication that works through a different mechanism than GLP-1 receptor agonists, and some patients may benefit from this alternative or even a combination approach under medical supervision. Having multiple medication pathways available through a single provider simplifies the treatment journey for patients who may need to adjust their protocol.

The main weakness in Tmates’ profile is its extremely limited review history. The platform’s Trustpilot page shows a 3.2 rating based on just 1 review, which provides essentially no useful data about the broader patient experience. Prospective patients should be aware that independent patient feedback is virtually nonexistent. The platform also does not offer lab testing or dietitian access, which keeps it from reaching the top tier of clinical comprehensiveness.

Despite the thin review history, Tmates presents a compelling package on paper: affordable pricing, insurance acceptance, a wide medication selection including brand-name options, and strong ongoing support. If the platform can build a more substantial body of patient reviews over time, it has the potential to move up in our rankings. For now, it earns a solid mid-range score that reflects genuine promise tempered by the need for more external validation.

From the Inside

What the Tmates Intake Looks Like

We walked through Tmates’ full GLP-1 intake ourselves, from the welcome screen through four sequential safety multi-selects to the no-account-required final checkout. Here’s what an actual prospective patient encounters before being prescribed.

24
Total Screens
~7 min
Time to Complete
10
Distinct Stages

Clinical Rigor
8
/ 10

One of the strongest compounded-provider intakes in the catalog. Four sequential safety multi-selects with rare granularity (cancer 5-year remission threshold, QT prolongation, HIV, low sodium, brain tumor), a dedicated opiate / substance use screen, BP and HR range checks, MTC + MEN2 family history called out by name, and a suicidal-thoughts checkbox. Knocked down by no validated PHQ instrument, no dedicated eating-disorder screen, no general allergy follow-up beyond Yes/No, and no medication list.

Friction Level
4
/ 10

Notably low. No early email gate, no mid-flow account creation, no ID upload, and no phone OTP. Name, email, and phone are collected only on the very last screen before “Proceed to Checkout” β€” meaning a prospective patient can complete the entire clinical questionnaire and see their plan options anonymously.

The 10 Stages

Stage 1
Welcome + Demographics
Welcome screen positions Tmates as “the #1 rated supplier of weight-loss medication online with over 40k users.” Then height + weight with live BMI, sex assigned at birth (Male / Female only), and DOB.
Stage 2
Safety Multi-Select #1
End-stage kidney disease (on or about to be on dialysis), end-stage liver disease (cirrhosis), suicidal thoughts / prior suicidal attempt, cancer with an explicit 5-year remission threshold (non-melanoma skin cancer carved out), severe GI condition (gastroparesis, blockage, IBD), current alcohol / opioid / substance use disorder.
Stage 3
Safety Multi-Select #2
Gallbladder disease, hypertension, seizures, glaucoma, sleep apnea, T2D not on insulin, T2D on insulin, T1D, and diabetic retinopathy.
Stage 4
Safety Multi-Select #3
Warfarin (Coumadin / Jantoven) use, pancreatitis history, “personal or family history of thyroid cyst / nodule, thyroid cancer, medullary thyroid carcinoma, or MEN2,” gout, high cholesterol or triglycerides, depression, head injury, brain or spinal cord tumor / infection, low sodium, liver disease (including fatty liver).
Stage 5
Safety Multi-Select #4
Tachycardia / elevated resting heart rate, CAD or heart attack / stroke in last 2 years, congestive heart failure, QT prolongation or other heart-rhythm disorder, hospitalization within last 1 year, HIV, acid reflux, asthma / reactive airway disease, urinary stress incontinence, PCOS, clinically proven low testosterone (more below the fold).
Stage 6
Allergy / Opiate / Surgery / Meds
Yes/No on medication allergies, a dedicated “within last 3 months, opiate pain meds and/or opiate-based street drugs?” Yes/No (rare β€” only Hims and Tmates observed), Yes/No on prior weight-loss surgeries, and Yes/No on currently taking prescription medications.
Stage 7
BP + HR Range
Blood pressure RANGE (<120/80, 120-129/<80, 130-139/80-89, β‰₯140/90 with AHA staging) and resting heart rate RANGE (<60, 60-100, 101-110, >110). Eligibility-check loading page leads to a weight-loss projection chart branded with Mayo Clinic, Harvard, Stanford Medicine, WebMD, NIH.
Stage 8
History + Interests
Past weight-loss medication within 4 weeks (GLP-1 / different / no), prior weight-management program (Jenny Craig, WW, etc.) Yes/No, free-text “anything else for the medical team,” and a wellness-style interests multi-select (muscle mass, no inject preference, side-effect management, longevity, cognition, energy, hormones, sleep).
Stage 9
State + Eligibility + Plan
State of residence dropdown. Eligibility reveal: “Congrats, you have been qualified for any of the plans below.” Dosage preference (Starter / Customize). Treatment selection: Injectable vs Oral, then Semaglutide ($199 monthly to $158/mo on 12-month) or Tirzepatide ($297 monthly to $167/mo on 12-month) with In Stock badges.
Stage 10
Final Checkout
“Results that last, with data to prove it” β€” money-back guarantee copy and a single screen collecting first name, last name, email, and phone. “Proceed to Checkout” button. No password, no account creation, no ID upload.

Clinical Safety Screens Performed

βœ“ Suicidal thoughts / prior attempt as a checkbox (no validated PHQ)
βœ“ Dedicated opiate / substance use 3-month screen (rare)
βœ“ Cancer with 5-year remission threshold (specific clinical wording)
βœ“ MTC + MEN2 personal AND family history (named explicitly)
βœ“ QT prolongation + heart rhythm disorder (rare)
βœ“ BP range + HR range with AHA-aligned categories
βœ“ Warfarin specifically named for drug-interaction screening
βœ“ HIV, brain tumor, low sodium in the multi-selects

⚠ What Tmates’ Intake Doesn’t Do

The intake doesn’t use a validated PHQ-2 or PHQ-9 depression screen β€” suicidal thoughts and depression are checkboxes in the safety multi-selects. There is no dedicated eating-disorder screen with severity grading (anorexia / bulimia were not visible in our walkthrough). The allergy capture is a single Yes/No on “allergic to any medications” with no follow-up list for food, dyes, supplements, or specific drugs. Current medications are a Yes/No only β€” no medication list is collected. There is no diet or exercise history beyond a Yes/No on prior weight-management programs (Jenny Craig, WW). Sex is collected as a binary Male / Female only.

πŸ’‘ The Whole Questionnaire Is Anonymous

Tmates is the only compounded GLP-1 provider in our catalog so far that lets a prospective patient complete the entire 24-screen clinical questionnaire β€” including all four safety multi-selects, BP and HR ranges, and the eligibility reveal β€” without entering an email, phone number, or creating an account. Contact information is collected on the very last screen right before “Proceed to Checkout.” For comparison, providers like Yucca Health drop an email gate at pre-qualification, and Strut Health requires email + phone (or Google SSO) before any clinical question. The trade-off: there’s no progress-saving link emailed to the patient mid-flow, so an interrupted session means starting over.
