Sesame Care’s defining feature is that the intake ends with the patient choosing a specific clinician from a real list. The “Pick your provider” screen shows multiple named clinicians with photos, 5-star ratings, specialty labels, next available appointment slots, and per-visit pricing. Filters allow selection by availability window, credentials (NP or MD), and provider specialty (including adult health, family medicine, internal medicine, and urgent care). The welcome screen also highlights ABOM certification (American Board of Obesity Medicine) as a provider filter — a real clinical credential that few other GLP-1 platforms surface to patients. Patients who want to see who they’re booking with before they book are likely to appreciate this model.

Throughout the lifestyle deep-dive, Sesame Care intersperses educational slides that cite actual clinical evidence. After the sleep question, a transition slide states: “Clinical studies show that getting less than 7 hours of sleep a night can increase the risk of weight gain by 15%.” After the exercise question: “A consistent diet and exercise routine is key to losing weight.” These aren’t marketing copy — they’re short, evidence-framed educational moments tied to the preceding answer. It’s a more clinical tone than most GLP-1 telehealth intakes.

Sesame Care is a marketplace, and its pricing reflects that. Final checkout charges an annual subscription of $708 upfront plus per-visit pricing “as low as $59/month” for the GLP-1 video appointments. The annual subscription covers Sesame’s platform fee and unlocks discounted rates across the marketplace, not just GLP-1 care. Included services in the program are explicitly listed at checkout: initial video consultation, free labs relevant to weight-loss treatment/diagnoses, pre-authorization paperwork coordination, and ongoing video visits with unlimited messaging. For patients who only want GLP-1 and don’t plan to use Sesame for other care, the subscription component is worth evaluating against single-specialty alternatives.

MTC and MEN2 are captured as personal history only — there is no explicit FDA-style family history screen for these conditions, which is a notable gap given that the FDA black-box warning applies to both personal and family history. The intake also does not include a substance-use or alcohol screen, a validated PHQ-2 or PHQ-9 depression instrument, an explicit suicide or self-harm question (depression and anxiety are listed in the broader conditions list but not screened directly), an actual blood pressure reading, an explicit pregnancy check, or a dedicated eating-disorder screen.
