Most intakes we’ve completed collect the same information but don’t tell the patient which answers would disqualify them until after clinician review. Three items in the Fridays gate are especially noteworthy: an explicit suicidal thoughts / prior attempt question, an explicit untreated substance use check (which specifically calls out binge drinking), and a diabetes-complexity check (Type 2 diabetes on insulin or A1c > 10%) that screens for patients whose diabetes may be too complex for a telehealth GLP-1 program. Few intakes we have completed include all three.

⚠ What the Fridays Intake Doesn’t Ask

It also doesn’t collect an actual blood-pressure reading, a validated PHQ-2 or PHQ-9 instrument (the suicidal thoughts question is binary), or perform ID verification or phone OTP.
