Most intakes we’ve completed collect the same information but don’t tell the patient which answers would disqualify them until after clinician review. Three items in the Fridays gate are especially noteworthy: an explicit suicidal thoughts / prior attempt question, an explicit untreated substance use check (which specifically calls out binge drinking), and a diabetes-complexity check (Type 2 diabetes on insulin or A1c > 10%) that screens for patients whose diabetes may be too complex for a telehealth GLP-1 program. Few intakes we have completed include all three.
Near the end of the flow, Fridays presents an unusually broad treatment menu: compounded tirzepatide (from $240), compounded semaglutide (from $150), oral liposomal compounded semaglutide ($150), oral liposomal compounded tirzepatide ($240), and — for patients who prefer brand-name product — branded Ozempic (limited supply, from $1,498) and branded Mounjaro (limited supply, from $1,928). The compounded tier includes both injectable and oral liposomal formats, which is rare. Plan tiers are then chosen between monthly auto-refill, 3-month starter bundle, 6-month “most popular,” and 12-month “best value,” with promo codes applied automatically.
The intake doesn’t include a prior-GLP-1-use question, a general drug / food allergy screen, a current medications review beyond the insulin check in the disqualification gate, or any diet or exercise history. It also doesn’t collect an actual blood-pressure reading, a validated PHQ-2 or PHQ-9 instrument (the suicidal thoughts question is binary), or perform ID verification or phone OTP. The short flow relies heavily on the single disqualification gate and the clinician review after checkout to fill in the depth.
